Agreement to Terms
These Terms of Service ("Terms") govern your use of the Legacy OBGYN website and services. By accessing or using our website, scheduling an appointment, or receiving our services, you agree to be bound by these Terms. If you do not agree to these Terms, please do not use our website or services.
Medical Services
Provider-Patient Relationship
A provider-patient relationship is established only when you have been examined by one of our healthcare providers. Use of this website or patient portal does not, by itself, create a provider-patient relationship.
Not for Medical Emergencies
This website and patient portal are not intended for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not use our website, patient portal, or email for urgent medical matters.
Appointment Scheduling
Online appointment requests are not confirmed until you receive a confirmation from our office. We reserve the right to reschedule appointments due to provider availability, emergencies, or other unforeseen circumstances.
Patient Responsibilities
As a patient, you agree to:
- Provide accurate and complete medical history and personal information
- Arrive on time for scheduled appointments
- Provide at least 24 hours notice for appointment cancellations or changes
- Bring your insurance card, photo ID, and payment method to appointments
- Follow treatment plans and recommendations provided by your healthcare provider
- Notify us of any changes to your contact information, insurance, or medical condition
- Treat our staff and providers with respect and courtesy
Cancellation and No-Show Policy
We understand that circumstances may require you to cancel or reschedule an appointment. Please notify us at least 24 hours in advance to avoid a cancellation fee.
- Late cancellations (less than 24 hours notice) may be subject to a $50 fee
- No-show appointments (failure to attend without notice) may be charged a $50 fee
- Repeated no-shows or late cancellations may result in dismissal from the practice
- Emergency situations will be considered on a case-by-case basis
Payment and Insurance
Financial Responsibility
You are financially responsible for all services provided. Co-payments are due at the time of service. If your insurance denies coverage, you are responsible for the full cost of services.
Insurance Verification
While we make every effort to verify your insurance coverage, it is ultimately your responsibility to understand your benefits, coverage limitations, and out-of-pocket costs. We recommend contacting your insurance provider to verify coverage before your appointment.
Payment Methods
We accept cash, checks, credit cards, and most major insurance plans. Payment plans may be available for large balances - please contact our billing department to discuss options.
Medical Records
Your medical records are confidential and protected under HIPAA regulations. You have the right to access and request copies of your medical records.
- Record requests must be made in writing
- A reasonable fee may be charged for copying and processing medical records
- Records will be released within 30 days of a valid request
- You must authorize in writing any release of records to third parties

Governing Law
