Digital Intake Forms
You will receive digital intake forms via email or text message prior to your appointment. Completing these forms in advance helps us prepare for your visit and saves time during check-in.
It's Normal to Feel Nervous
Many patients feel anxious before their first visit—that's completely normal! Our team is here to make you feel comfortable and answer all your questions. You're in control, and we'll never proceed with any exam without your consent. Your first visit may simply be a conversation to get to know you and discuss your health goals.
What to Bring
Required Documents
- Photo ID (driver's license or state ID)
- Insurance card (both sides)
- Payment method for co-pay

- Please complete your digital intake forms prior to your appointment to avoid delays during check-in
- If available, please bring any recent medical records related to your visit that have not already been provided to our office
- List of current medications and dosages
- Medical history (surgeries, allergies, chronic conditions)
- Family medical history (if known)
- First day of your last menstrual period
- List of questions or concerns

Important to Know:
- •Physical exams (breast, pelvic) may not be required on your first visit
- •Pap smears typically start at age 21 (not always needed at first visit)
- •You have the right to decline any exam or procedure
- •You're welcome to bring a support person to your appointment
Arrival and Check-In
New Patients: Arrive 30 Minutes Early
This allows time to complete check-in, verify insurance, and finish any remaining paperwork. If you've completed forms online in advance, you can arrive 15 minutes early.
During Check-In, We'll:
- Verify your personal and insurance information
- Collect co-payments (due at time of service)
- Help you register for our patient portal
- Answer any questions before your appointment
Insurance and Payment
What You Need to Know:
- Co-payments and deductibles are due at the time of service
- We accept most major insurance plans (see our Insurance & Billing page for details)
- We accept cash, checks, and all major credit cards
- If you don't see your insurance listed, call us to verify coverage

We recommend contacting your insurance provider before your visit to verify coverage, understand your benefits, and confirm your co-pay amount.
Office Policies
Cancellation Policy
We understand that sometimes circumstances require you to cancel or reschedule. Please notify us at least 24 hours in advance to avoid a cancellation fee.
- •Late cancellations (less than 24 hours) may incur a $50 fee
- •No-show appointments may be charged a $50 fee
- •Emergency situations will be considered on a case-by-case basis
Frequently Asked Questions
When should I schedule my first OB/GYN visit?
ACOG recommends young women have their first visit between ages 13-15, and no later than age 21. You should also schedule a visit if you're sexually active, experiencing menstrual concerns, need contraception, or planning a pregnancy.
Will I need a pelvic exam at my first visit?
Not necessarily. Many first visits are simply conversations to get to know you and discuss your health history. Your provider will discuss what exams, if any, are appropriate for your age and situation.
Can I bring someone with me to my appointment?
Absolutely! You're welcome to bring a parent, partner, friend, or support person to any appointment. They can wait in the waiting room or join you in the exam room—it's your choice.
Should I schedule during my period?
If possible, try to schedule when you're not menstruating, as this can affect exam results. However, if your concern is period-related or it's the only time you're available, please don't let this stop you from scheduling.
How long will my first appointment take?
Plan for approximately 45-60 minutes for your first visit, including check-in time. This allows plenty of time to get to know you and address all your questions and concerns.
After Your Visit
After your appointment, you'll have access to visit summaries and test results through our patient portal. If you need to schedule a follow-up appointment, our front desk can help you before you leave.
