SCHEDULING YOUR MAMMOGRAM.

You can schedule your routine mammogram at a time convenient for you by calling the Breast Center at DeKalb Medical Center (404) 501-5678 or go online www.dekalbmedicalcenter.org. If you would like to have your mammogram done at the Hillandale location, tell them when scheduling your appointment.

If you would like to have your mammogram done at another facility, call the main number of that facility and ask for the mammography department.

Tell them to send a copy of you mammogram report to our office. If your insurance requires you to have a primary care physician, have the facility send a copy of your mammogram report his or her office also.

VERY IMPORTANT:

Check with your insurance company to see if you will need a referral from our office or your primary care physician's office. The phone number for your insurance company is located on your insurance card. If you need a referral go ahead and schedule your mammogram appointment, then call our office or your primary care physicians office for the referral. When you call for the referral, we will need the name of the facility and date for your appointment.

IF A REFERRAL IS REQUIRED, YOUR INSURANCE WILL NOT PAY FOR THE MAMMOGRAM UNLESS YOU HAVE ONE. IF YOU FAIL TO GET A REFERRAL , YOU WILL BE RESPONSIBLE FOR THE BILL. WE CANNOT GET A RETROACTIVE REFERRAL FOR A SERVICE THAT HAS BEEN RENDERED.
